What it is used for
Tadagra (tadalafil) is taken by adult men for:
- Erectile dysfunction: improves erectile response with sexual stimulation.
- BPH symptoms: may reduce urinary hesitancy, weak stream, and frequency related to enlarged prostate.

How to take it
Always follow your prescriber's instructions. Swallow tablets whole with water. Try to take it at roughly the same time if you are on a daily regimen.

- On-demand for ED: 10 to 20 mg taken about 30 to 60 minutes before sexual activity; do not exceed once daily.
- Once-daily dosing: commonly 2.5 to 5 mg for men who prefer chronic use or for BPH; your clinician may adjust based on response and tolerability.

Precautions
Before starting tadalafil, tell your clinician if any of the following apply:
- History of cardiovascular disease, chest pain with exertion or sex, arrhythmias, or heart failure.
- Kidney or liver impairment.
- Penile anatomical deformities or conditions that predispose to priapism.
- Sudden changes in vision or hearing, or a history of those problems.
- Low baseline blood pressure or episodes of fainting.

Do not use if
Do not take Tadagra (tadalafil) if any of the following are true:
- You use any nitrate medicines (for example, nitroglycerin) or amyl nitrite. Serious, potentially dangerous drops in blood pressure can occur.
- You take riociguat for pulmonary arterial hypertension.
- Allergy to tadalafil or any tablet component.
- Unstable cardiovascular disease, recent heart attack within 90 days, or stroke within 6 months.
- Severe liver disease or marked hypotension.
- Hereditary eye disorders such as retinitis pigmentosa.
This product is not intended for women or for anyone under 18 years of age. If you are uncertain whether a condition applies to you, review the interactions and speak with your prescriber.
Possible side effects
Many effects are mild and short-lived. Contact a clinician if they are severe or persistent.

Less common: flushing, muscle aches, or visual color tinge. Stop use and seek urgent care for chest pain, sudden vision or hearing loss, or erections lasting more than 4 hours.
Drug and food interactions
Always provide your clinician and pharmacist a full list of prescription drugs, OTC medicines, vitamins, and herbal supplements before starting tadalafil.
- Nitrates or amyl nitrite: absolutely contraindicated due to profound blood pressure drop.
- Riociguat: do not combine.
- Alpha blockers and antihypertensives: additive blood pressure lowering may occur; dose separation and monitoring may be required.
- Strong CYP3A4 inhibitors (for example, ketoconazole, itraconazole, clarithromycin, ritonavir): can raise tadalafil levels; dose adjustment or avoidance may be needed.
- CYP3A4 inducers (for example, rifampin, carbamazepine, phenytoin): may reduce effectiveness.
- Alcohol: increases dizziness and orthostatic hypotension; use modest amounts if at all.
- Grapefruit or grapefruit juice: may increase tadalafil levels; discuss with your clinician.
If you have a surgery or procedure scheduled, inform your care team you use tadalafil.
Missed dose
Daily regimen: take the missed dose when remembered if the next dose is not due soon. Skip it if it is almost time for the next dose. Do not double up.
On-demand regimen: you take a dose only before sexual activity, so there is no scheduled dose to make up.
Overdose
Too much tadalafil can cause severe dizziness, fainting, or prolonged erection. Call your clinician, go to the nearest emergency department, or contact Poison Control in the U.S. at 1-800-222-1222. If symptoms are severe, call 911 immediately.
Storage
Keep tablets in the original blister or bottle at room temperature, protected from heat, moisture, and direct light. Store out of reach of children and pets. Do not use past the expiration date.
Alternatives and similar medicines
